How to Create Custom Forms in SteepleMate

Creating custom forms in SteepleMate allows you to gather important information from church members, guests, or volunteers. Whether you need forms for event registrations, feedback, or volunteer sign-ups, this guide will walk you through the process of creating, customizing, and managing your forms.


Step 1: Access the Form Management Tool

  1. Navigate to Forms

    • From the left navigation menu, click on Forms.
    • Then click Manage Forms to access the form builder.
  2. Create a New Form

    • Click New to start creating a new form.

Step 2: Define Form Properties and Details

  1. Select a Template

    • Choose a template you’d like to use for your form, or start from scratch. You can also upload images, define requirements, and adjust visibility settings.
  2. Set Form Visibility

    • Define who can access the form. For example:
      • Set visibility to None for public access.
      • Check the box for Add to Menu to make the form publicly available via your church's website or app.
  3. Save Your Settings

    • Once you've completed the initial setup, click Update to save your form details.

Step 3: Customize Form Fields

  1. Edit Form Fields

    • To edit or add form fields, click the > icon next to the form to view its details.
  2. Add a New Field

    • Click New to add a new field to the form.
    • Specify the order in which this field will appear on the form (e.g., if there are already four fields, you might set this as the 5th question).
  3. Customize Field Properties

    • Field Name: Choose an existing custom field, or click the link in the description to create a new custom field.
    • Field Width: Define how many characters wide the field should be.
    • New Row: Specify if the field should appear on a new row on the form.
    • Required Field: Set whether this field should be mandatory for form submission.
  4. Save Field Changes

    • After configuring the field, click Update to save your changes.

Step 4: Manage and Publish Your Form

  1. Manage Form Settings
    • You can further manage your form by:
      • Editing or Deleting form fields.
      • Viewing Submissions to see responses.
      • Filling Out the Form to test how it looks and functions.
      • Duplicating the Form for reuse.
      • Generating a QR Code to share the form easily.
